HEALTH FORM health form AND CURRENT IMMUNIZATION RECORDS IN COMPLIANCE WITH
MASSACHUSETTS BOARD OF HEALTH REGULATIONS. THESE ARE TO BE SUBMITTED TO
NEWTON RECREATION DEPARTMENT. IF YOU SIGN UP FOR MULTIPLE PROGRAMS IT MUST
BE DONE EACH TIME.
